The registration form allows you to register entirely online in phases as indicated by the checkmarks at the top of the form:

• Health History

• Permission to Participate

• Payment
   

In order for this process to run smoothly we recommend you have all your health information, dates and contacts in front of you before you start this process. Due to collecting all information in one spot, the registration form may take a few extra minutes to complete.

For those who have difficulties with the form, we can always complete it for you by sending in a paper copy to ucc@mnwcucc.org.
