New this year we will be implementing an paper free registration process! The registration form allows you to register entirely online in phases as indicated by the checkmarks at the top of the form:

In order for this process to run smoothly we recommend you have all your health information, dates and contacts in front of you before you start this process. Due to collecting all this information in one spot, this registration form may take a few extra minutes to complete.

For those who have difficulties with this form, we can always complete it for you by sending in a paper copy of this form to ucc@mnwcucc.org.
